Why did the accident occur.  What were the principal, main, chief, prime, or crucial reason the accident occurred.

Design: The condition existed in the original design.

Misapplication: The primary failure was the vessel was used for another purpose other than its original design.

Construction and Installation: The primary cause of the failure is the result of a condition that occurred during the construction or building of the vessel.

Maintenance: Had proper maintenance been completed the failure would not have occurred.

Repair:  The primary conditions for a failure was created during a past repair.

Operations:  The primary cause of the failure is related to how the vessel was used, started, and operated including being shutdown such that in normal and reasonable operation the failure would not have occurred.  This would include overloads, fast starts, improper procedures, etc..

External: The primary failure happened outside the vessel and under normal operations the vessel would still be operating.
